2011/2012 FUOYE Clearance And Registration Details

Fuoye has opened their gates for resumption since. So, if you are yet to get cleared, then know that the physical clearance will begins as from the 10th of April, 2012.



Things required of you as a candidate


Certificate of Local Government origin.



Acceptance Letter, Fees and Receipt


You must present the original of the following documents at the Clearance Centre. Documents like Letter of Admission by JAMB

JAMB result slip and other credentials.

Certificate of medical fitness obtained from any government hospital.

Birth certificate or sworn declaration of age.

4 recent coloured passport – size photographs withyour name and course of study clearly written
on the plain side.

Letter of attestation from your parent/guardian about
your good behaviour and conduct.

Student Bio-data form from the school website.


Thereafter, candidates are to present three (3) copies of each of the items in 1-10 above to theScreening Officer in the Academic Affairs Division.

STEP 2: After the Clearance officer must have certified that the candidate meets the general and course entry
requirements, he/she shall pay the appropriate fee using his/her DEBIT CARD. (You are advised not to share your
card details with anyone- this is very important!)

Thereafter, the Bank Receipt should be exchanged for the University Bursary Receipt

PLEASE VISIT THE SCHOOL WEBSITE HERE FOR DETAILED PROCEDURE ON THE USE OF DEBIT CARD FOR PAYMENT OF PRESCRIBED SERVICE CHARGES.

STEP3: All candidates shall present at the clearance centre the original and 3 photocopies of the university Bursary
Receipt for the payment of Service Fee and obtain the following.
a. Acceptance Form (FUOYE/ACAD/01)
b. Certificate of Clearance (FUOYE/ACAD/02)
c. Course Registration Form (FUOYE/ACAD/03)
STEP 4: All candidates shall:
a) Complete all the listed forms mentioned in step 3;
b) Make four (4) photocopies of the completed Acceptance, Certificate of clearance, Course Registration, form, Student’s Personal Data Form and submit them to the Clearance Officer. The Clearance Officer shall sign the Certificate of Clearance afterall the required documents have been completed and submitted.
Step 5: All candidates shall proceed to their respective Faculty Officer for course registration.
Step 6: At the Departmental office, candidates shall be provided the list of courses for first and second semesters.
He/she shall enter the appropriate courses in the first and second semester course enrollment forms and submit them at his/her Departmental Office.
